I hereby apply to become a member of Sydney Women's International Club Inc. In the event of my admission as a member, I agree:

1. To be bound by the current Constitution.
2. To adhere to the Club's policy regarding the confidentiality of all membership information contained in any written or digital material circulated by SWIC – including that:
- it must not be used for any purpose not related to the general function of the Club without written approval of the Committee.
- it must not be passed on to any person who is not a member of the Club for any purpose whatsoever.
- it must not be used by anyone for any commercial purpose whatsoever.
3. That a welcoming profile and my contact details be included in the Membership Updates and in the Member Directory on the Club's website www.swic.org.au (accessible by SWIC members only)*
4. That any photo taken of me at a SWIC activity may be posted on the Gallery in the members' area of the SWIC website, with your name but no other personal identification (accessible by SWIC members only)*
5. To receive a monthly email alert advising of activities/events in the coming month(s).
6. To sign a RISK WAIVER (below) in order to participate in activities that may expose me to risk that could lead to injury, illness or death or to loss of or damage to my property.

*NOTE: The Club has a secure members' only area on its website. The Committee will consider an Applicant's exceptions to 3 and 4 in the Declaration above, on an individual basis.

To be signed by members and visitors participating in activities that may expose them to risks that could lead to injury, illness or death or to loss or damage to their property.

I voluntarily participate in SWIC (Sydney Women's International Club Inc.) activities, and am aware that this may expose me to risk that could lead to injury, illness or death or to loss of or damage to my property. These risks may include, but are not limited to, slippery and/or uneven surfaces, rocks being dislodged, falling at edges of cliffs or drops or elsewhere, risks associated with crossing creeks, hypothermia and heat exhaustion. To minimise these risks, I will endeavour to ensure that any activity in which I participate is within my capabilities and that I am carrying food, water and equipment appropriate for the activity.

I agree to advise the activity leader if I am taking any medication or have any physical or other limitations that might affect my participation in the activity. I confirm that I am over 18 years of age. I will make every effort to remain with the rest of the party during the activity, and to accept the instructions of the leader of the activity.
